Fissures can be filled with hyporeflective layered keratin
SKs can also appear without surface ridges in the flat form
Keratin cysts are more noticeable in this form as hyperreflective structures embedded in the thickened epidermis
Flat SKs can often be similar in appearance to junctional nevi in terms of DEJ presentation
These can be distinguished from nevi by identifying hyperreflective structures within the epidermal thickening corresponding to keratin cysts and deposits
