  • Heterogeneous texture a result of dyskeratosis
  • Appearance of repeating ridges and fissures on the skin surface mirrored by the DEJ contour

  • Fissures can be filled with hyporeflective layered keratin

  • SKs can also appear without surface ridges in the flat form
  • Keratin cysts are more noticeable in this form as hyperreflective structures embedded in the thickened epidermis

  • Flat SKs can often be similar in appearance to junctional nevi in terms of DEJ presentation
  • These can be distinguished from nevi by identifying hyperreflective structures within the epidermal thickening corresponding to keratin cysts and deposits
